11/26/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C met with Ashiyah for an individual session. Ashiyah appeared to be in a good mood evident by her facial expressions such as a smile on the face as well as engaging in conversation with QMHP-C about her day. Ashiyah reported her day was well. QMHP-C informed Ashiyah that todayâs session will focus on increasing Ashiyahâs verbal and non-verbal communication skills. 2. Behavior: Ashiyah participated in the session and did not need any redirection from QMHP-C. Ashiyah was educated on the importance of her communicating her feelings to eliminate symptoms of depression and other behaviors that causes her to become upset and engage in the behavior she engages in. 1. Intervention: QMHP-C informed Ashiyah that the session would focus on ways to assist Ashiyah in developing techniques to increase her verbal and non-verbal communication skills in order to effectively express her thoughts and feelings due to Ashiyah having difficulties in expressing her feelings to certain individuals. QMHP-C introduced a therapeutic activity to Ashiyah in which it was processed with Ashiyah that it was designed to teach different techniques such as deep breathing with positive visual imagery that would help her to stay calm during certain situations. QMHP-C instructed Ashiyah to come up with positive words to use when she wants to express and communicate with others. Ashiyah was taught basic communication skills to improve her ability to express her thoughts, feelings, and needs more clearly. QMHP-C introduced another activity to Ashiyah. QMHP-C engaged Ashiyah in a conversation that would help her see who she had a closer bonding with and why. QMHP-C observed Ashiyah while she wrote how she felt about everyone she listed in her circle. 2. Intervention: QMHP-C engaged Ashiyah in a conversation that would help her see who she had a closer bonding with and why. QMHP-C provided warm acceptance and active listening techniques were utilized to establish the basis for a trust relationship with Ashiyah while discussing why she picked those certain individuals. QMHP-C also did this same activity with Ashiyah in order to show her that every individual wonât have the same people in the circle of trust. Through the use of role-modeling and role-playing, QMHP-C showed Ashiyah how to approach these various individuals to effectively express her feelings. QMHP-C gave Ashiyah the opportunity to choose situations that sheâs dealt with before in which she didnât effectively express her feelings in the correct manner. QMHP-C reviewed the objectives of the session with the Ashiyah and processed the information from the session. 1. Response: Ashiyah told QMHP-C that sheâs been doing good lately with expressing her feelings, especially towards her mother. She stated that her mother will pull her aside and talk about anything with her in order to keep their communication positive. She stated that in the past, she would be scared to tell her mother certain things because she thought she would get into trouble. Ashiyah participated in the deep breathing exercise but she didnât really take it too seriously as she stated that she didnât think it would really calm her down when presented with certain situations. 2. Response: Ashiyah had difficulties coming up with positive words to use when wanting to communicate with others. However; after QMHP-C gave her some examples, she was able to grasp the concept and give some of her own. Ashiyah listed her mother, her father, this QMHP-C, and two of her friends as to people she felt she could tell most things too. Ashiyah said that sheâs noticed that people she thought were her friends, they really arenât and this is why she doesnât communicate with them like she used to. Ashiyah presented a lot of humor when the role-modeling and role-playing was being implemented. QMHP-C Interpretation of clientâs progress towards todayâs objectives: Ashiyah continues to struggle with being able to express her feelings and emotions in an assertive manner. QMHP-C Plan of Action: VACCD staff will continue to meet with Ashiyah to continue to assist her self-monitoring and to address problematic behaviors outlined in her service plan.
